I, the data subject, by completing the form on the website lazafade.uz (hereinafter — the "Website"), freely, of my own will and in my own interest, give consent to TOJIBOYEV ABDULLAZIZ ABDULATIF O'G'LI, 51807017040065 (hereinafter — the "Operator") to process my personal data on the following terms:

I consent to the processing of the following personal data:

name (or alias);
phone number;
the name of the master I am booking with;
date of the appointment;
type of service;
cost of the service;
IP address, cookie data, information about activity on the Website, browser type and operating system (automatically transmitted data).

Purposes of processing personal data

Personal data is processed for the following purposes:

booking me for barbershop services;
contacting me regarding bookings and service delivery;
keeping records of services provided;
improving the quality of the Website.

List of actions with personal data

I consent to the Operator performing the following actions (operations) with my personal data: collection, recording, systematization, accumulation, storage, clarification (updating, amendment), retrieval, use, transfer (disclosure, access), depersonalization, blocking, deletion and destruction of personal data, both with and without the use of automation tools.

I grant the Operator the right to carry out cross-border transfer of my personal data to the territory of foreign states for the purpose of ensuring the functioning of the Website and the online booking service.

Validity period of this consent

This consent is valid from the date it is given until the purposes of personal data processing set out in this Consent are achieved, unless otherwise provided by the legislation of the Republic of Uzbekistan.

I have been notified and understand that I have the right to withdraw this consent at any time by sending a written notice to the Operator's email address: tadjibaev001@icloud.com or by sending a written application to the Operator's registered address.

If I withdraw my consent, the Operator is obliged to stop processing my personal data and destroy it within a period not exceeding thirty days from the date the withdrawal is received, unless otherwise provided by the legislation of the Republic of Uzbekistan.
